하악 제3대구치 발치 후 발생한 하치조신경 및 설신경 손상에 관한 연구

Abstract

이 연구의 목적은 치과의료분쟁 중 가장 많은 부분을 차지하고 있는 하악 제3대구치 발치수술 후 발생할 수 있는 하치조신경 및 설신경 지배부위의 감각이상에 대한 대책을 마련하기 위한 기초자료를 마련하기 위한 것이다. 14,000명의 치과의사에게 인터넷을 이용한 설문조사를 실시하여 2,472명의 치과의사에게 답변을 받았다. 이 설문조사의 결과에 근거하여 하악 제3대구치 발치 후 발생한 하치조신경 및 설신경 손상의 발생율 및 치과의사들의 경험율, 감각이상이 발생 후 해소되는 시기, 1년 이상 감각이상이 해소되지 않는 경우의 감각이상의 정도가 환자의 일상생활에 미치는 영향과 분쟁 발생에 따른 치과의사의 배상시기와 배상액등이 집계되었다. 그 결과, 현재 하악 제3대구치를 발치하고 있다고 응답한 치과의사 중 구강외과를 전공한 치과의사 수는 261명이었으며 구강외과를 전공하지 않은 치과의사의 수는 2,028명이었다. 구강외과를 전공한 치과의사 중 최근 1년간의 하악 제3대구치 발치 후 발생한 하치조신경의 감각이상을 경험한 치과의사의 비율은 20.7%, 설신경의 감각이상을 경험한 치과의사의 비율은 7.7%였다. 그에 비해 구강외과를 전공하지 않은 치과의사의 경험율은 각각 9.7%와 5.5%였다. 또한 구강외과를 전공한 치과의사 중 치과의사 면허를 취득한 이래 현재까지 하악 제3대구치를 발치한 후 하치조신경의 감각이상을 경험한 비율은 68.6%, 설신경의 감각이상을 경험한 비율은 29.1%였다. 그에 비해 구강외과를 전공하지 않은 치과의사의 경험율은 각각 37.3%와 14.1%였다. 이렇게 구강외과를 전공한 치과의사에게서 감각이상의 경험율이 더 높은 것은 이는 구강외과를 전공한 치과의사들이 하악 제3대구치의 발치수술의 횟수가 3.6배 정도 많았을 뿐 아니라, 하악 제3대구치의 단순 발치 보다 신경손상의 가능성이 훨씬 높은 골내 매복된 하악 제3대구치의 발치수술의 횟수가 많았을 것이 그 이유로 사료된다. 실제로 구강외과 전공 치과의사들은 최근 1년간 721개의 하악 제3대구치를 발치하였을 때 한 개의 비율로 하치조신경의 감각이상을 경험한 것이었으며 그 비율은 0.14%였다. 구강외과 비전공 치과의사의 경우는 최근 1년 간 531개의 하악 제3대구치를 발치하였을 경우 한 개의 비율로 하치조신경의 감각이상을 경험하였고 그 비율은 0.19%였다. 또한 설신경 지배 부위의 감각이상을 경험한 구강외과 전공 치과의사들은 1,938개의 하악 제3대구치를 발치하였을 때 한 개의 비율로 설신경의 감각이상을 경험하였고 그 비율은 0.05%였으며, 구강외과 비전공 치과의사들의 경우 설신경의 감각이상은 944개의 하악 제3대구치를 발치하였을 경우 한 개의 비율로 설신경의 감각이상을 경험하였고 그 비율은 0.11%였다. 하악 제3대구치의 발치 후 하치조신경 및 설신경의 감각이상은 하악 제3대구치의 해부학적 위치로 인하여 치과의사의 과오 없이도 발생할 수 있는 합병증으로 그 발생율이 높은 편이다. 이런 이유로 환자와의 의료분쟁의 단일 사유로 가장 높아 하악 제3대구치 발치수술은치과의사들이 꺼리는 시술이 되어가고 있어, 이에 대한 대책이 시급한 편이며, 이번 조사가 감각이상에 대한 치과의사와 환자간의 분쟁을 최소화하기 위한 객관적이고 합리적인 대책을 세우는 계기가 되기를 바란다.

This retrospective study was to analyze the inferior alveolar nerve and lingual nerve damage after the removal of mandibular third molars. In this questionnaire study, the subjects chosen for this study were 2472 dentists who answered the questionnaire about numbness after the extraction of lower third molars. The data collected by E-mail and web site included the incidence of removal of the lower third molars, the incidence and the experience of numbness of the inferior alveolar nerve and lingual nerve, rate and duration of recovery, the influence in day life after the long-term sensory loss, the period and amount of the indemnity in the case of medical dispute. The results are summarized as follows. 1. The experience rate and the incidence rate of the inferior alveolar nerve numbness by oral surgeons in the past year were 19.9% and 0.14%. Those of the lingual nerve by oral surgeon were 7.7% and 0.05%. 2. The experience rate and the incidence rate of the inferior alveolar nerve numbness by the dentists except oral surgeons in the past year were 9.7% and 0.19%. Those of the lingual nerve by the dentists except oral surgeons were 5.5% and 0.11%. 3. The recovery rate of the inferior alveolar nerve after 1 year and 2 years were 85.6% and 91.3%. The recovery rate of the lingual nerve after 1 year and 2 years were 84.8% and 89.3%. In conclusion, most of numbness may be recovered within 2 years. However the possibility of long term and persistent numbness should not be neglected. Therefore practitioner must inform the possibility of nerve injury and include this possibility in the consent forms.
